Mentre nei precedenti film Blade si era trovato invischiato in conflitti all'interno del mondo notturno dei vampiri, in questo episodio il più famoso Dhampyr (metà uomo metà vampiro) fumettistico da cacciatore diventa cacciato. I vampiri agiscono contro di lui su due fronti, da una parte manipolando l'FBI e l'opinione pubblica per far apparire Blade un pericoloso criminale, dall'altra risvegliando un antico vampiro primordiale per combatterlo efficacemente. Gli unici alleati di Blade sono i Nightstalkers, un gruppo di cacciatori di vampiri, che tra l'altro sta cercando di creare un virus per distruggere gli eredi di Dracula.

Blade is a day-walking vampire hunter. When the Vampire Nation hatches a plan to frame Blade in a series of brutal killings, he must join forces with the Nightstalkers, a clan of human vampire hunters, in an extreme battle in which the trail of blood leads directly to the notorious vampire legend, Dracula.
